Functionality, psychosocial factors and quality of life in women with predominance of central sensitization. Chronic pain is a major reason for visits to healthcare professionals and has been seen as a public health problem. Many patients with chronic pain may develop predominance of central sensitization.

The role of central sensitization in refractory pain-related diseases has not yet been clarified. We performed a multicenter case-controlled study including patients with various neurological, psychological, and pain disorders and 5, healthy controls to investigate the impact of central sensitization in these patients. Patients were categorized into 5 groups based on CSI-A scores from subclinical to extreme.

Metrics details. The identification of central sensitization CS is an important aspect in the management of patients with chronic musculoskeletal pain. Several methods have been developed, including clinical indicators and psychophysical measures. However, whether clinical indicators coincide with the psychophysical test of CS-related sign and symptoms is still unknown. Therefore, the present study aimed to analyze the diagnostic accuracy of the clinical indicators in identifying CS-related sign and symptoms in patients with musculoskeletal pain. One-hundred consecutive patients with musculoskeletal pain were included. Clinical indicators index method based on a combination of patient self-report pain characteristics and physical examination were used to identify the phenotype of patients with musculoskeletal pain and the predominance of the CS-related sign and symptoms.

Central NP is the result of a central lesion or disease such as stroke, multiple sclerosis or spinal cord injury [2] , whereas peripheral NP occurs from dysfunction or damage to peripheral nerves [3]. A study in Canada reported that A study in the United States US revealed that the prevalence rates for NP determined by either clinical examination or self-reporting were 9. It is difficult to obtain a true estimate, due to epidemiological studies using different methods of assessment and different definitions of NP [8]. A recent systematic review of epidemiological NP studies across the world by van Hecke et al.

Hyperexcitability of the central nervous system plays an important role in the development and maintenance of chronic pain in adults. This knowledge has led to improved treatment strategies within this population. In children, however, research on the presence of central hyperexcitability is scarce. To further investigate this topic in children with chronic pain, there is a need for a clear literature overview. The literature search was performed using the electronic databases PubMed and Web of Science. Articles had to report original research outcomes related to central hyperexcitability, and a comparison with a healthy control group was necessary. Characteristics of the study sample, the assessment, and conclusions regarding central hyperexcitability were extracted from each included article.

Different neuroplastic processes can occur along the nociceptive pathways and may be important in the transition from acute to chronic pain and for diagnosis and development of optimal management strategies. The neuroplastic processes may result in gain sensitisation or loss desensitisation of function in relation to the incoming nociceptive signals. Such processes play important roles in chronic pain, and although the clinical manifestations differ across condition processes, they share some common mechanistic features. The fundamental understanding and quantitative assessment of particularly some of the central sensitisation mechanisms can be translated from preclinical studies into the clinic. The clinical perspectives are implementation of such novel information into diagnostics, mechanistic phenotyping, prevention, personalised treatment, and drug development. The aims of this paper are to introduce and discuss 1 some common fundamental central pain mechanisms, 2 how they may translate into the clinical signs and symptoms across different chronic pain conditions, 3 how to evaluate gain and loss of function using quantitative pain assessment tools, and 4 the implications for optimising prevention and management of pain.

Previous reviews have reported that manifestations of pain sensitization may play an important role in the pain experienced by people with knee osteoarthritis. However, it is unknown if manifestations of pain sensitization are common features across other painful knee disorders or if sensitization requires targeted intervention. This review aims to synthesize the published research investigating manifestations of pain sensitization in painful knee disorders and to evaluate if the manifestations of pain sensitization change in response to treatment. We searched Medline, Embase, CINAHL, Web of Science, Sportsdiscus, and Cochrane Central for studies that investigated between-group differences knee pain vs pain-free subjects or the effect of treatment on manifestations of pain sensitization. Two reviewers independently assessed studies for inclusion and quality.

Chronic pain is pain that lasts a long time.

Assessment and manifestation of central sensitisation across different chronic pain conditions
