Kwashiorkor is the severe protein deficiency in the children, while marasmus is the severe deficiency of the nutrients and insufficient intake of calorie. This is the primary difference between the two. Protein-energy malnutrition PEM or Protein-calorie malnutrition PCM is the result of the deficiency of the carbohydrates, fats and especially proteins. The above two diseases, which are kwashiorkor and marasmus, are the commonest among such deficiencies and is frequently occurs in infants and children between 1 — 5 years of age. This is the reason for the mortality rate among children below the age of 5 years and even their sickness.

It can be distinguished from kwashiorkor in that kwashiorkor is protein deficiency with adequate energy intake whereas marasmus is inadequate energy intake in all forms, including protein. This clear-cut separation of marasmus and kwashiorkor is however not always clinically evident as kwashiorkor is often seen in a context of insufficient caloric intake, and mixed clinical pictures, called marasmic kwashiorkor, are possible. Protein wasting in kwashiorkor generally leads to edema and ascites , while muscular wasting and loss of subcutaneous fat are the main clinical signs of marasmus.

Symptoms of kwashiorkor

The relationship between protein energy malnutrition PEM and malaria is controversial. While most studies demonstrate that PEM is associated with greater malaria morbidity, some indicate that PEM may in fact have a protective effect. None of the studies concerning PEM and malaria seem to distinguish between these subgroups, and significant differences in susceptibility to malaria between these subgroups may have been overlooked. Plasmodium parasites and malaria infected erythrocytes are sensitive to oxidative stress. Since kwashiorkor patients seem to display an excess of prooxidants and as serum albumin is an important antioxidant, we hypothesized that patients with different forms of PEM might have different levels of malaria parasitaemia. There was no correlation between serum albumin and parasitaemia.
